Provide Informed Consent

1 In the menu bar, click References > Informed Consent.

2 Type your practice’s telephone number, and then click Next.

Note: This telephone number is checked against the Quest Diagnostics sales database. If your practice’s telephone number is not found, contact your sales representative.

Alternatively, you can click the link to download a PDF file of the PAIC form, complete and sign the form, and fax it to the fax number that appears on the page.

3 If any of the displayed information is incorrect, type the correct information.

Notes:   • To sign as an individual practitioner, you must provide an NPI. • If none of the information is correct, close this window and repeat this procedure, making sure that the telephone number is correct.

4 Type your name in the form, providing your first name first, followed by your last name.

5 Click the appropriate option button, indicating whether you are signing as an individual or on behalf of the entire practice, or if you prefer to sign an informed consent form for each applicable test.

6 Click Save Consent.

The page displays information about all of the clients covered by this form. When you have finished viewing this information, close the window.
