Add or Edit Guarantor Information

1 In the Guarantor area, do one of the following:

If no information appears, click Add Guarantor Information.

If the displayed information is incorrect, click Edit.

Note: Any SSN in the Guarantor area is masked except for the last four digits. To see or change this number, click Edit and then click in the SSN box on the Edit Guarantor Information dialog box. However, if Relationship is set to Self in the Order Details area, you cannot see or change this number unless you change the relationship.

2 Provide the appropriate information about the guarantor.

If Relation is set to Self in the Order Details area, then Relationship is automatically set to Self on the Edit Guarantor Information dialog box, the patient’s demographic information automatically appears, and you cannot change the SSN (if any) unless you click Spouse or Dependent in the Relationship list.

If you select the Use Patient’s Address check box, the patient’s address and telephone number appear automatically, and any changes that you make to the address or telephone number on this dialog box automatically update the order. Likewise, any changes that you make to the patient’s address or telephone number in the order automatically update the guarantor information.

If you clear this check box, any changes made to the address or telephone number here are not applied to the order, and any changes made to the address or telephone number in the order are not applied to the guarantor information.

3 Click OK.

4 You can optionally add comments to the order (as described in Add Comments). Otherwise, click Next and go to one of the following sections, depending on what is appropriate for this order:

Verify Insurance Eligibility

Specify a Diagnosis

Specify a Test

Notes:  

Eligibility verification occurs only if your user account was configured by an administrator to perform this action, and Bill Type is set to Insurance, and the patient’s primary or secondary carrier has provided eligibility data.

A diagnosis might be required, depending on the client or insurance carrier.
