Submit Feedback
1 Do one of the following:
• In the information bar, click Help > Feedback.
• At the bottom of any eLabs page, click the Feedback link.
• If a message with a Feedback link appears on the login page, click that link to provide feedback about that message.
2 If your name, telephone number, and email address do not appear automatically, or if any of that information is incorrect, type the appropriate information in the Name, Phone, and Email boxes.

Tip: To clear the information on the form and redisplay the default values (if any) at any time, click Reset. (This button does not appear if you accessed the Feedback Form dialog box from the login page.) |
3 If you would like someone from Quest Diagnostics to contact you regarding your feedback, select the Contact Me check box. Otherwise, leave this check box cleared.
4 In the Subject list, click the aspect of the product for which you are providing feedback (Ordering, Viewing Results, or Other).

Note: If you accessed the Feedback Form dialog box from the login page, the Subject list does not appear; the message from the login page appears instead. |
5 If you accessed the Feedback Form dialog box from the login page, in the Rating list, click the option that best describes your feedback (Positive, Negative, or Neutral).
6 In the Comments box, type your feedback.
7 Click Submit.
