Provide Template Data
1 If the Additional Information section includes a Templates area, provide the appropriate data.
The order code and test name associated with each template appear next to the template name. If two or more tests use the same template, only one instance of that template appears. The same responses are used for all applicable tests.
The data that you are prompted for varies, depending on which template is associated with each test in the order.

Notes: • For tests associated with the AmeriPath Oral Head and Neck Pathology template, the requisition will include a diagram where you can manually mark the specimen site. • For tests associated with the Cytogenetics/DNA Genetics Testing template, if you select a check box followed by a black asterisk (*), provide additional information in the Details box. |
