Provide a Preauthorization Code

1 If the Additional Information section includes a Test Pre-Authorization area, type the preauthorization code for each listed test.

If you do not have a preauthorization code, leave this box blank. You can add the code later (as described in Add or Change a Preauthorization Code, Managing Held Orders, or Managing Scheduled Orders).

2 Provide the order information (as described in Provide Order Information).
