Working with Ireland Exception Reports

Certain Quest Diagnostics labs perform testing for the National Cancer Screening Service (NCSS) in Ireland. The orders are placed at a Quest Diagnostics facility in Ireland, the specimens are sent to a lab in the United States for testing, the results are sent back to the facility in Ireland, and from there the reports are mailed to the clients.

Each order is supposed to have a unique accession number and other necessary information. However, under some circumstances (for example, if the order includes a test associated with a template, or if the web service that provides accession numbers is not available), no accession number is assigned. In addition, information that is not currently required to complete an order but that is required by the NCSS (such as NPP information) can be inadvertently omitted during order entry.

To see which orders are missing required information, you can request an Ireland exception report. The information in this report can help you address potential problems. For example, if NPP information is missing, you can edit the order via the requisition log and add it. If an accession number is missing, you can cancel the order via the requisition log and resubmit it.

Any orders that do not have all of the information required by the NCSS are omitted from the notification file that is sent to that organization each night. Orders that are omitted from the notification file are flagged and will not be included in any subsequent exception reports. Therefore, it is important to request an exception report before the notification file is generated.

Ireland Exception Report Content

The Ireland exception report is generated as a PDF file. Each page of the report contains the following information:

Date and time that the report was generated (yyyymmdd hh:mm:ss)

Name of the person who requested the report (Lastname, Firstname)

Filter criteria specified for the report

Interface message

Site ID and name of the performing lab

Client name and number

Errors that occurred

User name of the person who filed the order

Requisition number

Patient’s last name, first name, middle initial, address, date of birth, room number, patient ID, telephone number, and alternate ID (if available)

Date that the specimen was collected

Any comments included in the order

NPP information

Note: The Date Requested column in the Generated Reports area on the Report Manager page displays the date and time that the request to generate the report was submitted; the report itself reflects the date and time when the report generation was complete. Depending on the amount of data in the report, these dates or times could be different.
