Verify Insurance Eligibility
1 Do one of the following:
• To display the Eligibility Query page in your current browser window, in the menu bar, click Utilities > Eligibility Verification.
• To display the Eligibility Query page in a separate window or tab, in the information bar, click Tools > Eligibility Verification.

Note: These menu items do not appear for contracted phlebotomists. For PSC employees and IOPs, they might not appear for some labs. |
2 In the Performing Site list, click the lab that is performing the test.
This list is sorted by state and city, and includes the lab’s address, telephone number, and site ID. Your default lab appears in the list automatically. (For information about changing your default lab, see Change the Lab.)
3 In the Insurance Carrier list, click the patient’s insurance carrier.

Note: If the selected lab does not support eligibility verification, no carriers appear in this list. |
4 To verify eligibility for a date other than today, in the Date of Service box, type the appropriate date, or click the date on the calendar.
This date cannot be in the future.

5 Type information about the policy holder in at least one of the following boxes:
• Member’s Social Security
• Member Number
• Member’s Last Name
If you specify the policy holder’s last name, you can optionally specify the first name as well.

Note: Searching by the policy holder’s name works only when the policy holder resides in the same geographical region as the lab. If searching by name does not display any eligibility information, try searching by Social Security Number or insurance ID. |
You can see the Social Security Number as you type it, but it becomes masked as soon as you move to another field, and only the last four digits are visible. To see the number again, click in the Member’s Social Security box.
6 In the Member’s Birth Date box, type the policy holder’s date of birth.
7 To see any IPA message related to the policy holder’s zip code, type the zip code in the Member’s Zip Code box.
8 To see any client-specific IPA message, type the client number in the Client Number box.
9 Click Query.
The Response area displays the eligibility information. For more information about the eligibility statuses, see Eligibility Verification.
In addition, if the patient is not covered according to the terms of the carrier’s sub-plan, an IPA message appears below the Status message on this page.
