Provide Pap Data

1 If the Additional Information section prompts you for Pap data, provide the appropriate information.

Depending on the lab, one of two different Pap templates might appear.

Notes:   • For fields marked with a black asterisk (*), additional information is requested but not required. • To obtain the accession number from a previous Pap test, you can search the requisition log.
